Skilled medical home care services are designed to treat individuals in the safety of their own homes. Home care has been proven to result in optimal healing and recovery from illness or surgery. By establishing in home care services, patients can shorten their stay in hospitals and other acute care settings.
PARC Home Care’s staff of medical professionals works closely with your doctors and other health professionals to ensure that all clinical and medical needs are addressed. Our efficient referral and intake process ensures that a clinical staff member sees most patients within 24 hours.

Skilled Nursing
Our dedicated nurses manage patients' injuries, illnesses, and chronic conditions through skilled medical care including vital sign monitoring, wound and ostomy care, catheter care, and diabetes and chronic heart failure management.
Physical Therapy
Our licensed therapists help patients restore mobility and improve personal safety through fall prevention, muscle re-education, orthopedic and joint replacement care, and neurological rehabilitation for conditions such as Parkinson's, MS, and CVA.
Occupational Therapy
Our therapists assist patients in regaining independence at home through activities of daily living, upper body re-education, fine motor skill development, adaptive equipment training, splinting, and transfer training.
Speech Therapy
Our credentialed therapists assess, diagnose, and treat speech, communication, and swallowing disorders through cognitive therapy, memory training, swallowing treatment, and care for dysphagia and dysarthria.
Opioid Step-Down Program
Our nurses and physical therapists work collaboratively with patients, families, physicians, and pharmacists to reduce opioid dependence by identifying the lowest effective dosage and alternative non-medicinal pain management strategies focused on physical therapy.
Additional Support Services
To support each patient's overall plan of care, we provide access to medical social workers, home health aides, registered dietitians, community resources, and translation services as needed.
Fall Prevention Program
Falls are the leading cause of injury in adults over 65. Our clinicians specialize in evidence-based balance programs and fall risk assessments to create individualized prevention plans that help patients remain safe at home and out of the hospital.
Fast Track Orthopedic Program
Patients can transition from home care into our outpatient facility within one week, benefiting from continuity of care by working with the same physical therapist throughout their recovery.
In-Home Health Care Services
Program Goals include:
- Transition patients to their homes in a safe manner
- Educate the patient and their family about precautions and guidelines related to surgery after-effects and their recovery

- Nurses and therapists initiate services within 24 hours of discharge from a hospital or inpatient rehabilitation facility
- Therapists assess patients’ safety and durable medical equipment for your home needs
- All programs are tailored to the physician/surgeon protocols based on the type of surgery performed
- Therapists provide hands-on care to improve all functional outcomes for the patient to return to independence
- Skilled nurses assess with post-operative clinical status, including edema control, pain management, incision healing, and monitoring signs and symptoms of infection
- Skilled nurses and therapists work closely with our Opioid Step-Down Program team to allow patients to safely discharge medications as appropriate
- Home health aides, social workers, and nutritionists are available as needed
